This scan is a detailed fetal well-being assessment using growth parameters, amniotic fluid evaluation, and Doppler studies. It is commonly done in the third trimester (after 28 weeks), especially in high-risk pregnancies, to monitor fetal health and detect complications like fetal growth restriction (IUGR), placental insufficiency, and fetal distress.


1. Fetal Well-Being Assessment (FWB)

This includes:
Fetal Growth Parameters – Measures BPD, HC, AC, FL, and EFW to check if the baby is growing properly.
Amniotic Fluid Index (AFI) – Assesses the amount of amniotic fluid (low or excess fluid can indicate problems).
Fetal Position & Movement – Ensures the baby is in the correct position for delivery and is active.
Fetal Heart Rate (FHR) – Monitors the baby’s heartbeat to check for fetal distress.


2. Doppler Ultrasound (Blood Flow Assessment)

Doppler ultrasound helps evaluate blood circulation between the baby, placenta, and uterus. It is used to detect fetal distress, growth restrictions, and placental insufficiency.

📌 Types of Doppler Studies Performed:

🔹 Uterine Artery Doppler:

  • Checks blood flow from the mother to the placenta.

  • Abnormal Doppler may indicate placental insufficiency, preeclampsia, or IUGR.

🔹 Umbilical Artery Doppler:

  • Assesses blood flow between the placenta and fetus.

  • High resistance or absent/reversed flow suggests fetal distress or placental dysfunction.

🔹 Middle Cerebral Artery (MCA) Doppler:

  • Evaluates blood flow in the baby’s brain.

  • Increased MCA blood flow may indicate fetal anemia or hypoxia (low oxygen supply).


When is This Scan Done?

📌 Between 28 to 40 weeks for fetal monitoring.
📌 More frequently in high-risk pregnancies, such as:

  • IUGR (Intrauterine Growth Restriction)

  • Gestational Hypertension or Preeclampsia

  • Diabetes in Pregnancy

  • Previous stillbirth or placental abnormalities

  • Multiple pregnancies (twins, triplets, etc.)


Preparation for the Scan:

🔹 No fasting required.
🔹 Drink water before the scan (a moderately full bladder helps with imaging).
🔹 Wear loose, comfortable clothing.


Procedure:

1️⃣ A gel is applied to the abdomen.
2️⃣ The transducer (probe) is moved over the belly to capture images.
3️⃣ Fetal growth measurements are taken.
4️⃣ Doppler imaging is used to evaluate blood flow.
5️⃣ The scan takes 20-40 minutes and is painless.
