A BPP (Biophysical Profile) with FWB (Fetal Well-Being) Scan is a third-trimester ultrasound that assesses the baby’s health, growth, and oxygen supply inside the womb. It is commonly done after 28 weeks, especially in high-risk pregnancies, to detect fetal distress, low oxygen levels, or potential complications.


1. Fetal Well-Being (FWB) Assessment

Fetal Growth Parameters:

  • Biparietal Diameter (BPD) – Head width

  • Head Circumference (HC) – Overall head size

  • Abdominal Circumference (AC) – Determines fetal weight

  • Femur Length (FL) – Measures thigh bone growth

  • Estimated Fetal Weight (EFW) – Predicts if the baby is growing properly

Amniotic Fluid Index (AFI):

  • Checks if there is enough amniotic fluid (low or high levels may indicate a problem).

Fetal Heart Rate (FHR):

  • Measures the baby’s heartbeat and any variations indicating distress.

Fetal Position & Placenta Location:

  • Confirms if the baby is in the correct position for delivery and checks for placental abnormalities (e.g., placenta previa).


2. Biophysical Profile (BPP) – 5 Key Parameters

The BPP score (out of 10) evaluates fetal health based on five factors:

When is BPP with FWB Done?

📌 Between 28 to 40 weeks for fetal monitoring.
📌 Recommended in high-risk pregnancies, including:

  • IUGR (Intrauterine Growth Restriction)

  • Decreased Fetal Movements

  • Oligohydramnios (Low Amniotic Fluid) or Polyhydramnios (Excess Fluid)

Preparation for the Scan:

🔹 No fasting required.
🔹 Drink water before the scan (a moderately full bladder helps with imaging).
🔹 Wear comfortable, loose clothing.


Procedure:

1️⃣ A gel is applied to the abdomen.
2️⃣ The transducer (probe) is moved over the belly to capture images.
3️⃣ Fetal growth measurements and BPP parameters are recorded.

5️⃣ If required, a Non-Stress Test (NST) may be done to check fetal heart rate patterns.
