MRI Cisternography – Scan Details & Precautions

MRI Cisternography is a specialized MRI scan used to visualize cerebrospinal fluid (CSF) spaces, cisterns, and pathways in the brain and spinal cord. It is a non-invasive alternative to CT cisternography or invasive myelograms.


Test Details

Purpose of MRI Cisternography

  • Detects CSF leaks (rhinorrhea or otorrhea) due to trauma, surgery, or spontaneous leaks.
  • Evaluates hydrocephalus (fluid buildup in the brain).
  • Diagnoses arachnoid cysts, meningocele, or meningoencephalocele.
  • Assesses brain tumors affecting CSF flow.
  • Helps in pre-surgical planning for CSF-related disorders.

How Does MRI Cisternography Work?

  • Uses high-resolution MRI sequences (T2-weighted imaging) to highlight CSF pathways.
  • May include contrast dye (Gadolinium) via lumbar puncture (intrathecal injection) in rare cases.

How Long Does It Take?

  • 30–60 minutes (longer if contrast is used).

Precautions Before the Test

🔹 Before the MRI Cisternography Scan

1️⃣ Fasting Requirement

  • Fasting is usually not required, but some protocols may need fasting for 4–6 hours.

2️⃣ Metal & Implant Restrictions

  • Remove all metal objects (jewelry, piercings, glasses, belts).
  • Inform your doctor if you have:
    • Pacemakers, aneurysm clips, cochlear implants, or metallic implants.

3️⃣ Pregnancy & Claustrophobia

  • Inform your doctor if pregnant (MRI is generally safe, but contrast may not be recommended).
  • If you have claustrophobia, discuss sedation options or open MRI.

4️⃣ Contrast Dye Precautions (If Used)

  • Intrathecal Gadolinium may be used for enhanced imaging, requiring a lumbar puncture.
  • Kidney function (GFR test) may be required for patients with kidney disease.

During the MRI Cisternography Scan

  • You will lie on a table that slides into the MRI machine.
  • The MRI scanner makes loud knocking sounds (earplugs or headphones are provided).
  • Breath-holding may be required for short periods.
  • If contrast is used, an intrathecal (spinal) injection may be given before the scan.

After the MRI Cisternography Scan

  • Resume normal activities unless sedated or given a lumbar puncture.
  • If contrast was used, drink plenty of water to flush it out.
  • Results take 1–3 days, reviewed by a radiologist and neurologist/neurosurgeon.

Common Findings & What They Mean

📉 Abnormal MRI Cisternography May Indicate:

  • CSF Leak (Spinal or Skull Base Leak) → Can cause persistent headaches, dizziness, or meningitis risk.
  • Hydrocephalus → Increased CSF pressure due to blocked drainage pathways.
  • Arachnoid Cyst → Fluid-filled sacs that may compress nerves or brain structures.
  • Meningocele or Meningoencephalocele → Herniation of brain or spinal membranes.
  • Brain Tumor or Cyst → Can obstruct CSF flow and increase pressure.
