A Contrast-Enhanced MRI (CE MRI) of the Cervical Spine is an advanced imaging test that provides detailed visualization of the neck region, including the vertebrae, spinal cord, nerves, intervertebral discs, and soft tissues using a gadolinium-based contrast dye. This contrast improves detection of tumors, infections, inflammation, nerve compression, and vascular abnormalities.


Test Details

Purpose of CE MRI Cervical Spine

  • Identifies herniated discs, degenerative disc disease, and spinal stenosis.
  • Evaluates nerve root compression (cervical radiculopathy) or spinal cord compression (myelopathy).
  • Detects tumors, metastases, or abnormal growths in the spinal cord or vertebrae.
  • Diagnoses multiple sclerosis (MS) lesions, myelitis, or other inflammatory conditions.
  • Helps differentiate between post-surgical scarring and recurrent disc herniation.
  • Assesses vascular abnormalities (e.g., arteriovenous malformations, aneurysms).

How is it Different from a Regular MRI?

  • Uses Gadolinium-Based Contrast Dye, which helps:
    • Highlight tumors, inflammation, and nerve damage.
    • Distinguish between scar tissue and active disease in post-surgical cases.

How Long Does It Take?

  • 45–60 minutes (may take longer with contrast preparation).

Precautions Before the Test

🔹 Before the CE MRI

1️⃣ Kidney Function & Allergy Check

  • Gadolinium contrast is generally safe, but inform your doctor if you have:
    • Kidney disease or reduced kidney function (GFR test may be required).
    • Previous allergic reactions to contrast dye.

2️⃣ Metal & Implant Restrictions

  • Remove all metal objects (jewelry, piercings, glasses, belts, hearing aids).
  • Inform your doctor if you have:
    • Metal implants, pacemakers, aneurysm clips, or artificial joints.
    • Cervical spine hardware from previous surgeries.

3️⃣ Pregnancy & Claustrophobia

  • Inform your doctor if pregnant (MRI is safe, but contrast may not be recommended).
  • If you have claustrophobia, discuss sedation options or open MRI.

4️⃣ Fasting & Hydration

  • Fasting for 4–6 hours may be required if contrast is used.
  • Drink water before and after the test to help flush out the contrast.

During the CE MRI

  • A contrast dye injection is given via an IV line (may cause a mild warm sensation).
  • You will lie still on a table that slides into the MRI machine.
  • The MRI scanner makes loud knocking sounds (earplugs or headphones are provided).

After the CE MRI

  • Resume normal activities unless sedated.
  • If contrast was used, drink plenty of water to flush it out.
  • Results take 1–3 days, reviewed by a radiologist.

Common Findings & What They Mean

📉 Abnormal CE MRI May Indicate:

  • Herniated Disc or Spinal Stenosis → Compression of nerves causing neck pain, arm numbness, or weakness.
  • Cervical Myelopathy → Spinal cord compression leading to balance issues, weakness, or tingling.
  • Degenerative Disc Disease → Age-related wear and tear of the spinal discs.
  • Multiple Sclerosis (MS) Lesions or Myelitis → Autoimmune damage to the spinal cord.
  • Tumors (Benign or Malignant) → Contrast-enhanced areas may indicate spinal cord or vertebral tumors.
  • Infections or Abscesses → Brightened areas with contrast indicate inflammatory or infectious processes.
  • Post-Surgical Changes → Differentiates between scar tissue and recurrent disc herniation.
