Triple-Phase CECT Abdomen

Triple-Phase CECT Abdomen (Contrast-Enhanced CT with Arterial, Venous, and Delayed Phases)

A Triple-Phase CECT Abdomen is an advanced contrast-enhanced CT scan that captures images in three different phases (arterial, venous, and delayed) to provide detailed evaluation of the liver, pancreas, kidneys, and vascular structures. This scan is particularly useful for detecting liver tumors, vascular abnormalities, and abdominal malignancies.

Indications for Triple-Phase CECT Abdomen:

🔹 Liver Tumors & Lesions – Hepatocellular carcinoma (HCC), liver metastases, hemangiomas.
🔹 Pancreatic Tumors & Cysts – Pancreatic adenocarcinoma, neuroendocrine tumors.
🔹 Vascular Abnormalities – Aneurysms, thrombosis, or vascular malformations.
🔹 Portal Hypertension & Cirrhosis – Detects varices, ascites, and hepatic dysfunction.
🔹 Renal Masses & Tumors – Differentiates between benign and malignant lesions.
🔹 Trauma Evaluation – Assesses bleeding and organ injuries.
🔹 Biliary & Gallbladder Diseases – Choledochal cysts, bile duct strictures, or tumors.

Phases of Triple-Phase CECT Abdomen:

1️⃣ Arterial Phase (20–30 sec after contrast injection)

  • Highlights hepatic arteries and detects hypervascular tumors (e.g., hepatocellular carcinoma, neuroendocrine tumors).

2️⃣ Venous Phase (60–70 sec after contrast injection)

  • Shows portal veins, liver parenchyma, and identifies hypovascular tumors (e.g., metastases).

3️⃣ Delayed Phase (3–5 min after contrast injection)

  • Detects washout of contrast in malignancies, assesses fibrosis, biliary system abnormalities, and renal pathology.

Procedure:

  1. Preparation:

    • Fasting for 4–6 hours before the scan.

    • Serum Creatinine Test to check kidney function before contrast administration.

    • May require oral contrast for better gastrointestinal visualization.

  2. During the Scan:

    • The patient lies on a CT scan table.

    • An IV contrast dye (iodine-based) is injected, and images are taken in three different phases.

    • The procedure takes 20–30 minutes.

Results & Interpretation:

Normal Abdominal Structures: No abnormal masses, tumors, or vascular issues.
🚩 Hepatocellular Carcinoma (HCC): Arterial enhancement with venous/delayed phase washout.
🚩 Liver Hemangiomas: Arterial enhancement with persistent delayed-phase contrast.
🚩 Pancreatic Tumors: Different enhancement patterns help distinguish adenocarcinoma from neuroendocrine tumors.
🚩 Renal Tumors: Differentiates benign cysts from renal cell carcinoma (RCC).
🚩 Portal Vein Thrombosis: Non-enhancing clot within the portal vein.
🚩 Abdominal Trauma: Active bleeding, organ lacerations, or hematomas.

Why is Triple-Phase CECT Abdomen Important?

Essential for detecting and characterizing liver, pancreatic, and renal tumors.
Improves accuracy in differentiating benign from malignant lesions.
Guides treatment decisions, including surgery, embolization, or chemotherapy.
