Metanephrine Test – Details & Precautions

The Metanephrine Test measures metanephrines (metanephrine & normetanephrine) in blood or urine to help diagnose pheochromocytoma and paraganglioma—rare tumors that produce excessive adrenaline and noradrenaline.


Details of the Metanephrine Test

Purpose

  • Diagnoses pheochromocytomas (adrenal gland tumors) and paragangliomas (nerve tissue tumors).
  • Detects excess adrenaline (epinephrine) and noradrenaline (norepinephrine) levels.
  • Evaluates high blood pressure (hypertension) that may be due to hormonal imbalances.
  • Monitors patients with a history of pheochromocytoma for recurrence.

Types of Tests

1. Plasma Free Metanephrine Test (Blood Test)

  • Measures metanephrine and normetanephrine in the bloodstream.
  • More sensitive for detecting pheochromocytomas.

2. 24-Hour Urine Metanephrine Test

  • Collects all urine produced in 24 hours to measure metanephrines.
  • Used when blood test results are unclear.

Precautions for the Metanephrine Test

Before the Test

Fasting:

  • Blood test: Fast for at least 8–10 hours before the test (only water is allowed).
  • Urine test: No fasting is required.

Dietary Restrictions (24–48 hours before test):

  • Avoid caffeine (coffee, tea, energy drinks, cola).
  • Avoid bananas, chocolate, vanilla, nuts, and alcohol as they can affect results.

Medication Restrictions:

  • Inform your doctor about blood pressure medications, antidepressants, and decongestants, as they can interfere with the test.
  • If possible, stop epinephrine, norepinephrine, dopamine, and amphetamine-based medications under doctor supervision.

Avoid Stress & Physical Activity (24 hours before test):

  • Stress, exercise, and anxiety can increase metanephrine levels.
  • Stay calm and avoid intense physical activity before the test.

During the Test

  • Blood test: A small sample is taken from a vein; mild pain or bruising may occur.
  • Urine test: Collect all urine over 24 hours in a provided container.

After the Test

Resume normal activities immediately.
Monitor blood pressure if you have a history of hypertension.
