SLA (Soluble Liver Antigen) Antibody Test

The SLA (Soluble Liver Antigen) Antibody Test is a blood test used to detect autoantibodies against the Soluble Liver Antigen (SLA). It is primarily used to diagnose Autoimmune Hepatitis (AIH), specifically Type 1 or Type 3 AIH.

Why is the Test Done?

A doctor may order this test if a patient has:
Persistent liver enzyme elevation (AST, ALT, ALP)
Fatigue, jaundice (yellowing of skin/eyes), and abdominal discomfort
Unexplained liver inflammation (hepatitis) or cirrhosis
Suspicion of Autoimmune Hepatitis (AIH) after ruling out viral hepatitis and other liver diseases

What Does the Test Detect?

  • SLA antibodies are highly specific for Autoimmune Hepatitis (AIH).
  • Found in 10–30% of AIH patients, even when other liver autoantibodies (ANA, SMA, LKM-1) are negative.
Antibody Associated Disease Significance
SLA (Soluble Liver Antigen) Antibody Autoimmune Hepatitis (AIH) Highly specific for AIH, present in severe cases
ANA (Antinuclear Antibody) AIH Type 1, Lupus Common in AIH Type 1, not specific
SMA (Smooth Muscle Antibody) AIH Type 1 Seen in AIH but also in other conditions
LKM-1 (Liver-Kidney Microsomal Antibody) AIH Type 2 More common in children

How is the Test Done?

  1. Blood Sample Collection:

    • A sample is drawn from a vein.
    • Sent to the lab for ELISA (Enzyme-Linked Immunosorbent Assay) or Immunoblot testing.
  2. Laboratory Analysis:

    • Detects SLA antibodies and their concentration in the blood.

Test Results Interpretation

Result Interpretation
SLA Positive Strong indicator of Autoimmune Hepatitis (AIH)
SLA Negative Less likely to have AIH, but other tests may be needed

🔬 Further Tests May Be Ordered:

  • Liver Function Tests (LFTs) – To check liver enzymes (AST, ALT, ALP, Bilirubin).
  • Autoimmune Panel (ANA, SMA, LKM-1) – To confirm AIH type.
  • Liver Biopsy – If AIH is strongly suspected.

Next Steps After a Positive Test

💊 Treatment may include:

  • Corticosteroids (Prednisone) – To suppress immune response.
  • Azathioprine or Mycophenolate Mofetil – For long-term management.
  • Liver monitoring – Regular LFTs and imaging.
