HLA-B27 PCR

HLA-B27 PCR Test

The HLA-B27 PCR test detects the presence of the HLA-B27 gene using polymerase chain reaction (PCR). HLA-B27 is a genetic marker associated with certain autoimmune diseases, particularly ankylosing spondylitis (AS) and other seronegative spondyloarthropathies.

Why is the HLA-B27 Test Done?

Diagnosing Ankylosing Spondylitis (AS) – A chronic inflammatory arthritis affecting the spine and sacroiliac joints.
Identifying Other Spondyloarthropathies, including:

  • Reactive Arthritis (ReA)
  • Psoriatic Arthritis (PsA)
  • Enteropathic Arthritis (associated with IBD – Crohn’s disease & ulcerative colitis)
  • Juvenile Idiopathic Arthritis (JIA), especially enthesitis-related arthritis (ERA)
Family Screening – Since HLA-B27 is hereditary, testing may be done in family members with symptoms.
Aiding in Autoimmune Disease Diagnosis – Can support clinical suspicion but is not diagnostic on its own.

HLA-B27 Test Results & Interpretation

Result Interpretation
Positive (+) HLA-B27 gene is present; higher risk of spondyloarthropathies but does not confirm disease.
Negative (-) HLA-B27 gene is absent; does not rule out the disease, but lowers the likelihood.

🔹 HLA-B27 is present in ~90% of people with ankylosing spondylitis, but not all individuals with HLA-B27 will develop the disease.

HLA-B27 & Associated Diseases

Disease HLA-B27 Positive Rate
Ankylosing Spondylitis (AS) 80–95%
Reactive Arthritis (ReA) 50–80%
Psoriatic Arthritis (PsA) 20–50%
Enteropathic Arthritis (IBD-Associated Arthritis) 30–50%
Juvenile Idiopathic Arthritis (JIA, Enthesitis-related) 50–85%

📌 Important: Having HLA-B27 does not mean you will develop a disease, just that your risk is higher.

HLA-B27 PCR vs. Other Tests

Test Purpose Use Case
HLA-B27 PCR Detects genetic predisposition Helps diagnose autoimmune diseases
ESR & CRP Measures inflammation Active inflammation in arthritis
X-ray / MRI (Sacroiliac Joints & Spine) Detects joint damage Confirms ankylosing spondylitis
Rheumatoid Factor (RF) & Anti-CCP Rules out rheumatoid arthritis Negative in HLA-B27 diseases

Key Takeaways:

HLA-B27 is a genetic marker linked to ankylosing spondylitis & other autoimmune diseases.
A positive test does not confirm disease; symptoms & imaging are needed for diagnosis.
It is useful for risk assessment, family screening, and supporting clinical evaluation.
Additional inflammatory markers & imaging are needed for a full diagnosis.
