Troponin-I

The Troponin-I test is a blood test used primarily to detect heart muscle damage, often due to a heart attack (myocardial infarction). Troponin-I is a protein found in heart muscle cells, and when these cells are injured, troponin-I is released into the bloodstream.

Why is the Troponin-I test done?

  1. Diagnose a heart attack – It helps confirm or rule out a heart attack if you have chest pain or other symptoms.
  2. Monitor heart conditions – It can detect heart damage from conditions like myocarditis (heart inflammation) or heart failure.
  3. Assess risk of heart disease – In some cases, it may help evaluate heart stress in critically ill patients.

Normal vs. Abnormal Troponin-I Levels

  • Normal: Usually < 0.04 ng/mL (varies by lab).
  • Borderline/Elevated: 0.04–0.39 ng/mL may indicate heart strain or minor damage.
  • High (Indicative of heart attack): ≥ 0.40 ng/mL suggests significant heart injury.

When is the test ordered?

  • Chest pain or discomfort
  • Shortness of breath
  • Nausea, dizziness, or sweating (heart attack symptoms)
  • Monitoring after heart surgery or heart disease diagnosis

What do high Troponin-I levels mean?

  • Heart attack (most common)
  • Myocarditis (inflammation of the heart)
  • Heart failure
  • Blood clot in the lungs (pulmonary embolism)
  • Kidney disease (can cause falsely high levels)

How is the test done?

  • A small blood sample is taken from a vein in your arm.
  • Results are often available within 1–2 hours in emergencies.
