The Anti-Proteinase 3 (PR3) Antibody Test detects autoantibodies against proteinase 3 (PR3), an enzyme found in neutrophils. It is primarily used to diagnose and monitor ANCA-associated vasculitis (AAV), particularly Granulomatosis with Polyangiitis (GPA, formerly Wegener’s granulomatosis).
✅ To Diagnose ANCA-Associated Vasculitis (AAV)
✅ To Differentiate Between Types of Vasculitis
✅ To Monitor Disease Activity & Treatment Response
🩸 Blood Sample Collection:
| Result | PR3 Antibody Level (U/mL) | Interpretation |
|---|---|---|
| Negative | < 9 U/mL | No significant PR3 antibodies detected |
| Borderline | 9–20 U/mL | Possible early disease, follow-up needed |
| Positive | > 20 U/mL | Suggests ANCA-associated vasculitis (especially GPA) |
🔹 A positive PR3-ANCA test does NOT confirm disease on its own; clinical correlation is needed.
🔹 Some patients with active vasculitis may test negative for PR3-ANCA.
🔴 High PR3-ANCA Levels Suggest:
🟢 Negative PR3-ANCA:

✅ Treatment for PR3-ANCA-Associated Conditions:
🔹 PR3-ANCA is a key marker for Granulomatosis with Polyangiitis (GPA).
🔹 A positive test should be correlated with clinical symptoms and other lab findings.
🔹 Early diagnosis & treatment can prevent severe organ damage.
🔹 Long-term monitoring is needed for disease recurrence.
