Desmoglein I Antibody Test

The Desmoglein I (DSG1) Antibody test detects autoantibodies against Desmoglein-1, a protein responsible for skin cell adhesion. This test is primarily used to diagnose Pemphigus Foliaceus (PF), an autoimmune blistering skin disorder.

Why is the Test Done?

✅ To diagnose Pemphigus Foliaceus (PF), a skin blistering disease
✅ To differentiate Pemphigus Foliaceus from Pemphigus Vulgaris (PV)
✅ To monitor disease progression and treatment response

Normal Range & Interpretation

Result Interpretation
Negative (<20 U/mL) No significant antibodies detected (Normal)
Borderline (20-30 U/mL) Possible mild or early disease
Positive (>30 U/mL) Strong indication of Pemphigus Foliaceus (PF)

What Does a Positive Desmoglein I Antibody Mean?

🔴 High levels (>30 U/mL) → Suggests Pemphigus Foliaceus (PF)
🔴 Desmoglein I & III Antibodies together → Indicates more severe disease (Mucocutaneous Pemphigus)
🔴 Monitoring levels → Helps track disease activity and response to treatment

Difference Between Pemphigus Foliaceus & Pemphigus Vulgaris

Feature Pemphigus Foliaceus (PF) Pemphigus Vulgaris (PV)
Desmoglein Antibody DSG1+ DSG3+ (sometimes DSG1+)
Blister Location Superficial (Skin only) Deep blisters (Mucosa + Skin)
Pain Level Mild to moderate Severe, especially in mouth
Nikolsky’s Sign Positive Positive

Symptoms of Pemphigus Foliaceus (PF)

âš  Superficial blisters on the face, scalp, and trunk
âš  Flaky, crusty skin lesions
âš  No involvement of the mouth or mucous membranes
âš  Skin peels easily (Positive Nikolsky’s Sign)

Additional Tests for PF Diagnosis

🔹 Desmoglein III Antibody Test – To rule out Pemphigus Vulgaris (PV)
🔹 Skin Biopsy with Direct Immunofluorescence (DIF) – Confirms antibody deposits
🔹 Indirect Immunofluorescence (IIF) – Detects circulating autoantibodies
🔹 ELISA for DSG1 & DSG3 – Quantifies antibody levels

Treatment of Pemphigus Foliaceus (PF)

✔ Corticosteroids (Prednisone) – First-line treatment
✔ Immunosuppressants (Azathioprine, Mycophenolate Mofetil) – To control autoimmunity
✔ Rituximab (Biologic Therapy) – Effective for severe cases
✔ Topical steroids & wound care – Helps manage skin lesions
