DCP (Decarboxy Prothrombin) / PIVKA-II Test

DCP (Des-γ-carboxy Prothrombin), also known as PIVKA-II (Protein Induced by Vitamin K Absence or Antagonist-II), is a biomarker used primarily for detecting hepatocellular carcinoma (HCC), the most common type of liver cancer.

Why is the DCP / PIVKA-II Test Done?

Detect and monitor Hepatocellular Carcinoma (HCC) (Liver Cancer)
Differentiate HCC from benign liver diseases like cirrhosis or hepatitis
Assess liver function in chronic liver disease (e.g., cirrhosis, hepatitis B or C)
Monitor response to liver cancer treatment

Normal Range of DCP / PIVKA-II (varies by lab and measurement unit)

Test Result Interpretation
< 40 mAU/mL Normal (low risk of HCC)
> 40 mAU/mL Elevated (possible liver cancer or liver disease)

🔹 Higher levels (>100 mAU/mL) strongly suggest HCC presence
🔹 Mildly elevated levels can be seen in cirrhosis or vitamin K deficiency

High DCP / PIVKA-II Levels – Causes

🔺 Hepatocellular Carcinoma (HCC) – Primary liver cancer
🔺 Chronic Liver Diseases (Cirrhosis, Hepatitis B or C) – May increase levels
🔺 Vitamin K Deficiency – Can cause a mild increase
🔺 Use of Vitamin K Antagonists (e.g., Warfarin) – Can falsely raise levels

📌 DCP is more specific for liver cancer than AFP (Alpha-Fetoprotein), another common HCC marker.

Additional Tests if DCP is High

🩸 AFP (Alpha-Fetoprotein) Test – Another marker for liver cancer
🩸 Liver Function Tests (LFTs) – To assess liver health
🩸 Ultrasound / CT / MRI of the Liver – To detect tumors
🩸 HBV & HCV Testing – To check for hepatitis infections

Next Steps if DCP / PIVKA-II is Elevated

Imaging tests (CT/MRI) to confirm liver cancer
Biopsy (if needed) to diagnose HCC
Regular monitoring if cirrhosis is present
Treatment planning (surgery, liver transplant, or targeted therapy)

DCP / PIVKA-II vs. AFP – Which is Better for Liver Cancer?

Marker Best for Sensitivity for HCC
DCP / PIVKA-II More specific for liver cancer, detects aggressive tumors Good (Detects early-stage HCC)
AFP (Alpha-Fetoprotein) Used for screening & monitoring HCC Variable (Can be high in non-cancer liver diseases)

📌 For best accuracy, doctors often use BOTH DCP & AFP tests together.

Treatment & Management

For Liver Cancer: Surgery, targeted therapy, or liver transplant
For Liver Disease: Antiviral treatment (for hepatitis), lifestyle changes
For Vitamin K Deficiency: Vitamin K supplementation
