Anti-Nuclear Antibody (ANA) Test: Overview

The Anti-Nuclear Antibody (ANA) test detects autoantibodies that target the nucleus of cells. It is commonly used to screen for autoimmune diseases, particularly systemic lupus erythematosus (SLE) and other connective tissue disorders.

Purpose of the Test

Screen for autoimmune diseases (especially SLE, Sjögren’s syndrome, and scleroderma)
Investigate unexplained joint pain, fatigue, rashes, and other autoimmune symptoms
Monitor disease progression and treatment response in autoimmune conditions

Interpreting Results

Result Interpretation
Negative No significant ANA detected (autoimmune disease unlikely)
Low Positive (1:40 – 1:80 titer) May be seen in healthy individuals, mild inflammation, or early autoimmune disease
Moderate to High Positive (≥1:160 titer) Suggests autoimmune disease, further testing needed

🔹 Low-positive ANA results may not be clinically significant, as some healthy individuals have ANA.
🔹 High ANA titers (≥1:160) are more likely associated with autoimmune diseases.
🔹 A positive ANA does not confirm a disease—additional tests are needed for diagnosis.

Common Autoimmune Diseases Associated with Positive ANA

Disease Typical ANA Patterns Other Antibody Tests
Systemic Lupus Erythematosus (SLE) Homogeneous, Speckled Anti-dsDNA, Anti-Smith (Anti-Sm)
Rheumatoid Arthritis (RA) Speckled Anti-CCP, Rheumatoid Factor (RF)
Sjögren’s Syndrome Speckled Anti-SSA/Ro, Anti-SSB/La
Systemic Sclerosis (Scleroderma) Centromere, Nucleolar Anti-Scl-70, Anti-Centromere
Mixed Connective Tissue Disease (MCTD) Speckled Anti-RNP
Drug-Induced Lupus Homogeneous Anti-Histone

Additional Tests After a Positive ANA

ANA Titer & Pattern Analysis → Higher titers and specific patterns can indicate different diseases.
Anti-dsDNA & Anti-Smith (Anti-Sm) → Specific for SLE.
Anti-RNP, Anti-SSA/Ro, Anti-SSB/La → Seen in MCTD and Sjögren’s Syndrome.
Anti-Centromere & Anti-Scl-70 → Help diagnose scleroderma.
Rheumatoid Factor (RF) & Anti-CCP → Check for rheumatoid arthritis.

Next Steps if ANA is Positive

🔹 Evaluate symptoms (joint pain, rashes, fatigue, dry eyes/mouth).
🔹 Perform additional antibody tests to identify specific autoimmune conditions.
🔹 Consult a rheumatologist for further assessment and possible diagnosis.
🔹 Monitor disease activity if diagnosed with an autoimmune disorder.
