NCCT Orbit (Non-Contrast CT Orbit) – Overview

NCCT Orbit is a non-contrast CT scan of the orbits (eye sockets) used to assess bony structures, soft tissues, and detect various orbital pathologies without the use of contrast dye. It is commonly used in trauma cases, fractures, infections, and orbital tumors.

1. Indications (Why It’s Done?)

🔹 Orbital Trauma – Fractures, foreign bodies, hemorrhage
🔹 Proptosis (Bulging Eye) – Retrobulbar hematoma, orbital cellulitis
🔹 Orbital Infections – Cellulitis, abscess formation
🔹 Optic Nerve Disorders – Optic neuritis, optic canal compression
🔹 Sinus Pathology Affecting the Orbits – Sinusitis with orbital extension
🔹 Tumors & Masses – Orbital lymphoma, metastases
🔹 Bony Abnormalities – Congenital deformities, fractures
🔹 Preoperative Planning – Before orbital surgery

2. Procedure & Technique

Patient Preparation

  • No special preparation is needed

  • Patient lies supine with head secured in a CT scanner

Scan Details

  • Type: Non-contrast CT (NCCT)

  • Slices: Thin-section axial images with coronal and sagittal reconstructions

  • Field of View (FOV): Focused on orbits, includes paranasal sinuses

  • Bone & Soft Tissue Windows:
    🔹 Bone Window – For fractures, bony changes
    🔹 Soft Tissue Window – For muscles, nerves, fat, and eye globe

3. Key Findings

✅ Trauma & Fractures

  • Orbital wall fractures (blowout fractures, zygomatic fractures)

  • Intraorbital hemorrhage (retrobulbar hematoma)

  • Foreign bodies (glass, metal, wood)

✅ Orbital Infections & Inflammation

  • Orbital Cellulitis: Swelling, fat stranding, proptosis

  • Preseptal Cellulitis: Soft tissue thickening anterior to the septum

  • Abscesses: Fluid collections with rim enhancement (if contrast is used)

✅ Tumors & Masses

  • Optic nerve glioma

  • Orbital metastases

  • Lacrimal gland tumors

✅ Optic Nerve & Globe Abnormalities

  • Optic nerve compression (due to tumors, trauma)

  • Bulging or atrophic globe

  • Retinal detachment

✅ Sinus Pathology Affecting Orbits

  • Chronic sinusitis with orbital extension

  • Mucoceles causing orbital compression

4. Advantages of NCCT Orbit

✔ Quick & readily available
✔ Excellent for detecting fractures & foreign bodies
✔ No contrast needed (safer for kidney patients & those allergic to contrast)

5. Limitations

❌ Limited soft tissue detail (MRI Orbit is better for optic nerve & tumors)
❌ Radiation exposure (though minimal)
