Multiple Myeloma Comprehensive Panel – Key Tests & Markers

This panel helps diagnose, monitor, and assess the prognosis of Multiple Myeloma (MM) by detecting abnormal plasma cell activity, monoclonal proteins, and disease progression markers.

πŸ§ͺ Blood & Urine Tests for Multiple Myeloma

1️⃣ Serum Protein Electrophoresis (SPEP) & Immunofixation

βœ” Purpose: Detects and identifies monoclonal (M) proteins produced by myeloma cells.
βœ” Findings:

  • M-spike (Monoclonal Protein Band): Sign of clonal plasma cell proliferation.

  • Polyclonal Gammopathy: Seen in infections, autoimmune diseases.

  • Hypogammaglobulinemia: Low immune function, common in MM.

2️⃣ Immunoglobulin Quantification (IgA, IgG, IgM Levels)

βœ” Purpose: Measures levels of major immunoglobulins.
βœ” Findings:

  • IgG or IgA Myeloma: Overproduction of a single type (most common forms).

  • IgM: Usually normal or low in MM (high levels suggest WaldenstrΓΆm’s Macroglobulinemia).

3️⃣ Beta-2 Microglobulin (B2M)

βœ” Purpose: Measures tumor burden and disease progression.
βœ” Findings:

  • Higher levels: Indicate poor prognosis and advanced disease.

  • Used in staging (Revised ISS system).

4️⃣ Free Kappa & Lambda Light Chain Assay

βœ” Purpose: Detects light chain imbalance, crucial for Light Chain Myeloma.
βœ” Findings:

  • Elevated Free Kappa/Lambda Ratio: Sign of monoclonal plasma cell disorder.

  • Light Chain (Bence Jones) Myeloma: Excess free light chains cause kidney damage.

5️⃣ Urine Protein Electrophoresis (UPEP) & Immunofixation

βœ” Purpose: Detects Bence Jones Proteins (free light chains in urine).
βœ” Findings:

  • Present in Light Chain Myeloma, a more aggressive form.

πŸ” Additional Tests for Multiple Myeloma Diagnosis & Monitoring

πŸ”Ή Complete Blood Count (CBC): Checks for anemia, thrombocytopenia.
πŸ”Ή Calcium Levels: High calcium due to bone breakdown.
πŸ”Ή Renal Function Tests (Creatinine, BUN): Evaluates kidney damage.
πŸ”Ή Lactate Dehydrogenase (LDH): Marker of aggressive disease.
πŸ”Ή Bone Marrow Biopsy: Confirms >10% abnormal plasma cells in marrow.
πŸ”Ή Whole-Body CT / MRI / PET-CT: Detects bone lesions, fractures, and tumor activity.

Why This Panel is Important?

βœ” Confirms Diagnosis – Differentiates MM from other plasma cell disorders.
βœ” Assesses Tumor Burden & Prognosis – B2M, Light Chains help in staging.
βœ” Monitors Treatment Response – Tracks changes in M-protein and light chain levels.
βœ” Detects Kidney & Bone Damage Early – Prevents complications.
