CT Urography (CTU) – Detailed Evaluation of the Urinary Tract

CT Urography (CTU) is a specialized contrast-enhanced CT scan designed to provide high-resolution imaging of the kidneys, ureters, and bladder. It is primarily used for evaluating urinary tract abnormalities, hematuria (blood in urine), stones, tumors, and congenital anomalies.


Indications for CT Urography:

🔹 Unexplained Hematuria (Blood in Urine) – Detects underlying causes like tumors, stones, or infections.
🔹 Urinary Tract Tumors – Evaluates cancers of the kidneys, ureters, or bladder.
🔹 Kidney Stones & Ureteric Obstruction – Identifies calculi and their effect on urine flow.
🔹 Recurrent Urinary Tract Infections (UTIs) – Checks for anatomical abnormalities.
🔹 Congenital Urinary Tract Anomalies – Assesses birth defects affecting kidney function.
🔹 Bladder Abnormalities – Detects diverticula, masses, or structural issues.
🔹 Postoperative or Trauma Evaluation – Assesses complications after surgery or injuries.


Phases of CT Urography:

1️⃣ Non-Contrast Phase (NCCT) – Initial Scan:

  • Detects kidney stones, calcifications, and masses before contrast administration.

2️⃣ Nephrographic Phase (90 sec after contrast injection):

  • Evaluates renal parenchyma, tumors, and kidney function.

3️⃣ Excretory (Delayed) Phase (5–10 min after contrast injection):

  • Visualizes contrast-filled ureters and bladder, highlighting strictures, tumors, and urothelial abnormalities.


Procedure:

  1. Preparation:

    • Fasting for 4–6 hours before the scan.

    • Serum Creatinine Test to assess kidney function (required before contrast use).

    • May require oral hydration to improve excretory phase imaging.

  2. During the Scan:

    • The patient lies on a CT scan table.

    • An IV contrast dye (iodine-based) is injected for enhanced imaging.

    • The scanner captures images in multiple phases (NCCT, nephrographic, and excretory phase).

    • The procedure takes 20–30 minutes.


Results & Interpretation:

Normal Urinary Tract: No stones, tumors, or obstructions.
🚩 Kidney Stones (Calculi): High-density structures seen in the non-contrast phase.
🚩 Urothelial Carcinoma: Irregular filling defects in the ureters or bladder.
🚩 Hydronephrosis: Swelling of the kidneys due to urine obstruction.
🚩 Bladder Tumors: Contrast-enhancing masses in the bladder wall.
🚩 Ureteric Strictures or Anomalies: Narrowing or abnormal development of the urinary tract.
🚩 Post-Trauma Findings: Kidney or bladder injury.
