Anti-Endomysial Antibody (EMA-IgM) Test

What is it?

The Anti-Endomysial Antibody (EMA-IgM) test detects IgM-class autoantibodies against endomysium, a connective tissue component in muscles. These antibodies are primarily associated with celiac disease, an autoimmune disorder triggered by gluten intake.

Clinical Significance:

  • Celiac Disease (Gluten-sensitive enteropathy) – EMA-IgM is less commonly used for celiac disease diagnosis, as EMA-IgA is more specific.

  • IgA Deficiency – In individuals with selective IgA deficiency, IgM-based tests may be helpful in detecting celiac disease.

  • Dermatitis Herpetiformis – A skin condition linked to gluten sensitivity, often associated with positive EMA antibodies.

Normal Range & Interpretation:

  • Negative: No detectable IgM EMA antibodies (normal result).

  • Positive: Suggests a possible autoimmune reaction to gluten, often seen in celiac disease but requires confirmation with further tests (such as anti-tissue transglutaminase (tTG) antibodies, intestinal biopsy, or genetic testing for HLA-DQ2/DQ8).

Why is this Test Ordered?

  • To help diagnose celiac disease, especially in cases of IgA deficiency.

  • To monitor dietary adherence in celiac disease patients.

  • To investigate unexplained gastrointestinal symptoms (diarrhea, bloating, malabsorption, weight loss)
