FWB + BPP + Doppler Ultrasound in Pregnancy

This ultrasound combines three important assessments to check fetal well-being and circulation in late pregnancy:

1️⃣ FWB (Fetal Well-Being Assessment)
2️⃣ BPP (Biophysical Profile)
3️⃣ Doppler Study

It is typically done in the third trimester (after 28 weeks), especially in high-risk pregnancies to monitor fetal health and detect complications.

1. FWB (Fetal Well-Being Assessment)

This includes:
Fetal Growth Parameters – Measuring BPD, HC, AC, FL, and EFW to check if the baby is growing properly.
Amniotic Fluid Levels (AFI) – Checking if there is enough amniotic fluid.
Fetal Position – Determining if the baby is in the head-down position for delivery.
Fetal Heart Rate (FHR) – Monitoring the baby’s heartbeat to detect distress.

2. BPP (Biophysical Profile Score – 5 Key Parameters)

The BPP score (out of 10) evaluates fetal health based on:

Parameter Normal (2 points) Abnormal (0 points)
Fetal Movements 3+ movements in 30 min Less than 3 movements
Fetal Tone Flexion & extension seen No movement or abnormal tone
Fetal Breathing 1+ breathing movements in 30 min No fetal breathing
Amniotic Fluid Index (AFI) Normal amount Too low or absent
Non-Stress Test (NST) (if included) Normal FHR pattern Abnormal HR pattern

🔹 Normal BPP Score: 8-10 (Healthy Baby)
🔹 Borderline BPP Score: 6 (Needs Monitoring)
🔹 Low BPP Score: ≤ 4 (Possible Fetal Distress – Immediate Action Needed)

3. Doppler Study (Fetal & Placental Blood Flow Assessment)

This checks blood circulation between the baby and placenta using Doppler ultrasound:

Umbilical Artery Doppler – Measures blood flow between placenta & baby.
Middle Cerebral Artery (MCA) Doppler – Checks fetal brain circulation for oxygen supply.
Ductus Venosus Doppler – Evaluates fetal heart & liver blood flow.
Uterine Artery Doppler – Assesses placental function and risk of preeclampsia.

🔹 Abnormal Doppler readings may indicate:
Placental insufficiency (poor oxygen/nutrient supply).
Fetal growth restriction (IUGR).
Risk of preterm birth or fetal distress.

When is FWB + BPP + Doppler Scan Done?

📌 Between 28-40 weeks for fetal monitoring.
📌 More frequently in high-risk pregnancies (diabetes, hypertension, IUGR, multiple pregnancy, previous stillbirth).

Preparation for the Scan:

🔹 No fasting required.
🔹 Drink some water before the scan (for better amniotic fluid assessment).
🔹 Wear loose clothing for easy access to the abdomen.

Procedure:

1️⃣ Gel is applied to the abdomen.
2️⃣ Fetal measurements (FWB) are taken.
3️⃣ BPP parameters are observed in real-time.
4️⃣ Doppler ultrasound evaluates blood flow patterns.
5️⃣ The scan takes 30-45 minutes and is painless.

Is This Scan Safe?

Yes, it is completely safe, non-invasive, and radiation-free.
✅ Helps prevent complications by detecting fetal distress early.
