Metanephrine Test – Overview

What is the Metanephrine Test?

The metanephrine test measures the levels of metanephrines (metanephrine & normetanephrine) in blood or urine. These are breakdown products of epinephrine (adrenaline) and norepinephrine, produced by the adrenal glands.

This test is primarily used to diagnose or rule out pheochromocytoma and paraganglioma, which are rare tumors that cause excess catecholamine production, leading to high blood pressure, palpitations, and sweating.

1. Types of Metanephrine Tests

🔬 Plasma Free Metanephrines (Blood Test)

  • Measures metanephrine and normetanephrine in the blood.
  • Highly sensitive and often preferred for initial screening.

💧 24-Hour Urine Metanephrines Test

  • Measures total metanephrine levels over 24 hours.
  • Used for confirming high plasma results or for monitoring.

2. Why is the Metanephrine Test Done?

To Diagnose Pheochromocytoma & Paraganglioma
To Investigate Uncontrolled Hypertension (especially if episodic and resistant to medication)
To Monitor Known Tumors (before and after treatment)

3. Test Procedure

📌 Plasma Free Metanephrines (Blood Test)

  • Fasting for 8-12 hours is recommended.
  • Rest in a calm, seated, or lying position for 15–30 minutes before the test.
  • Avoid caffeine, alcohol, and stress before testing.

📌 24-Hour Urine Metanephrines

  • Collect all urine for 24 hours in a special container.
  • Avoid foods like bananas, chocolate, coffee, and vanilla, which can affect results.

4. Normal & Abnormal Metanephrine Levels

Test Type Normal Range Elevated Levels Suggest
Plasma Metanephrine <0.5 nmol/L Pheochromocytoma, stress, kidney disease
Plasma Normetanephrine <0.9 nmol/L Same as above
24-Hour Urine Metanephrines <1.3 mg/day Pheochromocytoma, excess adrenaline production

🛑 Very high levels (>4x normal) are strongly suggestive of pheochromocytoma.

5. Causes of Elevated Metanephrines

🔹 Pheochromocytoma & Paraganglioma (Primary Cause)

  • Rare adrenal or extra-adrenal tumors that secrete excess catecholamines.
  • Symptoms: Severe high blood pressure, headaches, sweating, rapid heart rate, anxiety.

🔹 Other Causes of High Metanephrine Levels

  • Severe stress, pain, or anxiety
  • Intense exercise before the test
  • Hypertension (especially resistant cases)
  • Kidney or liver disease
  • Certain medications (antidepressants, amphetamines, decongestants, beta-blockers)

6. Treatment & Next Steps

If Results Are Elevated

  • Imaging Tests (CT/MRI scan) to locate pheochromocytoma.
  • Further testing (Clonidine Suppression Test) to confirm diagnosis.

If Pheochromocytoma is Confirmed

  • Surgical removal of the tumor is the primary treatment.
  • Medications (alpha-blockers & beta-blockers) to control blood pressure before surgery.

Conclusion

The metanephrine test is a crucial diagnostic tool for detecting pheochromocytoma and paraganglioma, which can cause life-threatening hypertension. If results are abnormal, further evaluation with imaging is necessary.
