Urine Amylase

Urine Amylase Test

The Urine Amylase Test measures the level of amylase, an enzyme that helps break down carbohydrates, in the urine. Amylase is primarily produced by the pancreas and salivary glands. This test is often used to evaluate pancreatic disorders, particularly acute pancreatitis.

Why is the Urine Amylase Test Done?

Doctors may order this test to:
Diagnose or monitor acute pancreatitis
Check for pancreatic or salivary gland disorders
Evaluate severe abdominal pain, nausea, and vomiting
Assess kidney function (as amylase is excreted by the kidneys)
Monitor chronic pancreatitis or pancreatic duct obstruction

How is the Test Done?

1️⃣ Random Urine Sample – A single urine sample is collected.
2️⃣ 24-Hour Urine Collection – Provides a more accurate assessment of amylase levels.
3️⃣ Lab Analysis – Measures amylase activity (U/L or U/hour).

🚫 Pre-Test Instructions

  • Avoid alcohol before the test (can affect pancreas function).
  • Stay hydrated to ensure proper kidney function.
  • Inform your doctor about medications (some drugs affect amylase levels).

Normal Urine Amylase Levels

  • 24-Hour Urine Sample: 2–17 U/h or 250–2000 U/L
  • Random Urine Sample: Varies based on hydration and collection time

📌 Note: Normal ranges may slightly differ between labs.

Interpreting Urine Amylase Results

🔺 High Urine Amylase (>2000 U/L in 24 hours)

  • Acute pancreatitis (high levels persist longer in urine than in blood)
  • Chronic pancreatitis (mild to moderate increase)
  • Pancreatic duct obstruction (e.g., gallstones, tumors)
  • Mumps or salivary gland inflammation
  • Kidney disease (delayed amylase clearance)

🔻 Low Urine Amylase

  • Pancreatic insufficiency (chronic pancreatitis, cystic fibrosis)
  • Kidney disease (reduced excretion of amylase)
  • Liver disease (affecting enzyme metabolism)

Next Steps If Abnormal

🔹 Serum Amylase & Lipase Test – To confirm pancreatic disorders
🔹 Abdominal Ultrasound or CT Scan – To check for pancreas abnormalities
🔹 Kidney Function Tests – If low amylase is suspected due to kidney disease
