Contrast-Enhanced Magnetic Resonance Angiography (CE-MRA) – Scan Details & Precautions

Contrast-Enhanced Magnetic Resonance Angiography (CE-MRA) is a specialized MRI scan that provides detailed imaging of blood vessels (arteries and veins) using a contrast agent (Gadolinium-based dye). It is used to diagnose vascular diseases, aneurysms, blockages, and abnormalities in blood circulation.

Test Details

Purpose of CE-MRA Scan

  • Detects narrowed or blocked arteries (stenosis) due to atherosclerosis.
  • Identifies aneurysms (bulging arteries) and arteriovenous malformations (AVMs).
  • Evaluates stroke risk, carotid artery disease, or brain vascular abnormalities.
  • Assesses aortic aneurysms, dissections, or renal artery stenosis.
  • Diagnoses deep vein thrombosis (DVT) or pulmonary embolism (PE).
  • Helps in pre-surgical planning for vascular surgeries.

Common CE-MRA Areas

  • Brain (Cerebral MRA) → Detects aneurysms, strokes, or AVMs.
  • Neck (Carotid MRA) → Evaluates carotid artery blockages (stroke risk).
  • Heart (Coronary MRA) → Assesses coronary artery disease (CAD).
  • Chest (Thoracic MRA) → Checks aortic aneurysms or dissections.
  • Abdomen (Renal & Mesenteric MRA) → Evaluates kidney and intestinal blood supply.
  • Legs (Peripheral MRA) → Diagnoses peripheral artery disease (PAD).

Why Use Contrast (Gadolinium)?

  • Improves visibility of small blood vessels and abnormalities.
  • Helps differentiate normal vs. diseased blood vessels.
  • Provides clearer images compared to non-contrast MRA.

How Long Does It Take?

  • 30–60 minutes (longer if multiple areas are scanned).

Precautions Before the Test

🔹 Before the CE-MRA Scan

1️⃣ Kidney Function & Allergy Check

  • Gadolinium contrast is safe for most patients, but kidney function (GFR test) may be required for those with kidney disease.
  • Inform your doctor about any contrast dye allergies.

2️⃣ Metal & Implant Restrictions

  • Remove all metal objects (jewelry, piercings, glasses, belts).
  • Inform your doctor if you have:
    • Pacemakers, aneurysm clips, cochlear implants, or metallic stents.
    • Artificial heart valves or deep brain stimulators.

3️⃣ Pregnancy & Claustrophobia

  • Inform your doctor if pregnant (MRI is safe, but contrast may not be recommended).
  • If you have claustrophobia, discuss sedation options or open MRI.

4️⃣ Fasting & Hydration

  • Fasting for 4–6 hours may be required before the scan.
  • Drink plenty of water before and after the test to flush out the contrast.

During the CE-MRA Scan

  • An IV line is inserted to administer Gadolinium contrast dye.
  • You will lie on a table that slides into the MRI machine.
  • The MRI scanner makes loud knocking sounds (earplugs or headphones are provided).
  • Breath-holding may be required for short periods.

After the CE-MRA Scan

  • Resume normal activities unless sedated.
  • If contrast was used, drink plenty of water to help flush it out.
  • Results take 1–3 days, reviewed by a radiologist and vascular specialist.

Common Findings & What They Mean

📉 Abnormal CE-MRA May Indicate:

  • Aneurysm (Bulging Artery) → Risk of rupture leading to stroke or internal bleeding.
  • Arterial Stenosis (Narrowing) → Can cause stroke, heart attack, or limb ischemia.
  • Arteriovenous Malformation (AVM) → Abnormal artery-vein connections leading to bleeding risk.
  • Aortic Aneurysm or Dissection → Life-threatening tearing of the aorta.
  • Carotid Artery Disease → Increases stroke risk.
  • Peripheral Artery Disease (PAD) → Poor blood flow in legs, leading to pain or ulcers.
