A Contrast-Enhanced MRI (CE MRI) of the Lumbar Spine is an advanced imaging test that provides detailed visualization of the lower back (lumbar region), spinal cord, nerves, intervertebral discs, and surrounding tissues using a gadolinium-based contrast dye. This contrast helps detect tumors, infections, inflammation, nerve compression, or post-surgical complications more clearly.


Test Details

Purpose of CE MRI LS Spine

  • Detects herniated discs, spinal stenosis, and degenerative disc disease.
  • Assesses nerve root compression (radiculopathy, sciatica).
  • Identifies tumors, infections, abscesses, or inflammatory conditions.
  • Evaluates spinal cord disorders such as multiple sclerosis (MS) or myelitis.
  • Detects post-surgical changes, scarring, or recurrent disc herniation.
  • Examines vascular abnormalities (e.g., spinal arteriovenous malformations).

How is it Different from a Regular MRI?

  • Uses Gadolinium-Based Contrast Dye to:
    • Highlight tumors, inflammation, and nerve damage.
    • Differentiate between scar tissue and a new disc herniation in post-surgical cases.

How Long Does It Take?

  • 45–60 minutes (may take longer with contrast preparation).

Precautions Before the Test

🔹 Before the CE MRI

1️⃣ Kidney Function & Allergy Check

  • Gadolinium contrast is generally safe, but inform your doctor if you have:
    • Kidney disease or reduced kidney function (GFR test may be required).
    • Previous allergic reactions to contrast dye.

2️⃣ Metal & Implant Restrictions

  • Remove all metal objects (jewelry, piercings, glasses, belts, hearing aids).
  • Inform your doctor if you have:
    • Metal implants, pacemakers, aneurysm clips, or artificial joints.
    • Spinal hardware from previous surgeries.

3️⃣ Pregnancy & Claustrophobia

  • Inform your doctor if pregnant (MRI is safe, but contrast may not be recommended).
  • If you have claustrophobia, discuss sedation options or open MRI.

4️⃣ Fasting & Hydration

  • Fasting for 4–6 hours may be required if contrast is used.
  • Drink water before and after the test to help flush out the contrast.

During the CE MRI

  • A contrast dye injection is given via an IV line (may cause a mild warm sensation).
  • You will lie still on a table that slides into the MRI machine.
  • The MRI scanner makes loud knocking sounds (earplugs or headphones are provided).

After the CE MRI

  • Resume normal activities unless sedated.
  • If contrast was used, drink plenty of water to flush it out.
  • Results take 1–3 days, reviewed by a radiologist.

Common Findings & What They Mean

📉 Abnormal CE MRI May Indicate:

  • Herniated Disc or Spinal Stenosis → Compression of nerves causing pain, numbness, or weakness in legs (sciatica).
  • Degenerative Disc Disease → Wear and tear of spinal discs leading to chronic back pain.
  • Post-Surgical Scar Tissue vs. Recurrent Herniation → Contrast helps differentiate between fibrosis (scarring) and new disc issues.
  • Tumors (Benign or Malignant) → Contrast-enhanced areas may suggest spinal cord or vertebral tumors.
  • Infections or Abscesses → Brightened areas with contrast indicate inflammatory or infectious processes.
  • Multiple Sclerosis (MS) Lesions or Myelitis → Abnormal enhancement in the spinal cord.
  • Spinal Vascular Malformations → Blood vessel abnormalities like arteriovenous malformations (AVMs).
